Athletic Forms
All student athletes are required to be cleared for athletic competition through a sports physical. The physical must be performed by a Doctor, Physician's Assistant, or Nurse Practioner. Per NJCAA rule, a physical is good for 13 months from its completion date.
Student athletes must also have Primary insurance when participating in intercollegiate athletics at St. Cloud Technical and Community College.
The below forms must also be completed and turned into the athletics office before a student will be able to participate in any athletic activity including tryouts and/or practices.
